In consideration for my attendance and participation in the Jiu Jitsu and self-defence training offered by Lucid Jiu Jitsu & Self-Defence, I, the student/parent/guardian, acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks. I further relieve the school, its management, assigned staff, and fellow students from any liability resulting from loss, whether personal belongings or bodily injury.
From time to time, images and videos may be taken of classes and events, which I agree the club can use for promotional purposes indefinitely. If I do not wish for my or my child’s image to be used, I will contact the club owner.
I understand Jiu Jitsu and Self Defence is a contact activity that carries with it a degree of risk of injury during training as well as close body contact at times. I hereby agree to follow the instructions of Lucid Jiu Jitsu & Self-Defence management, instructors, and staff, assuming all inherent risk associated with my or my child’s participation in this type of training.
I also hereby state that myself or my child is physically fit, and that I or my child have/has no medical conditions or injuries that would preclude participation. I am doing so of my own free will in exchange for an agreed upon fee.
I will immediately notify Lucid Jiu Jitsu & Self-Defence management if I become aware of any person with whom my child or I have had contact exhibits any symptoms of COVID-19, is advised to self-isolate, quarantine, or has tested positive, or is presumed positive for COVID-19.
I understand that participants will be in contact with children, families, and employees who are also at risk of community exposure. I understand that no list of restrictions, guidelines, or practices will remove 100% of the risk of exposure to COVID-19, as the virus can be transmitted by persons who are asymptomatic. I understand that I play a crucial role in keeping everyone in the facility safe by following these practices.
All names and addresses on our mailing list are stored on computer files. These files will not be passed to any other party, for advertising purposes or for any other reason. Should you object to having your name and address stored in this way, please inform our office and we will remove your entry. We do not store credit card details nor share customer details with any third parties.
We reserve the right to remove from a course, workshop, or event any person that is unable or unwilling to conduct themselves in a manner compatible with the satisfactory operation of the event or instructor. In this event, we will make no refund nor be liable to pay any compensation, and any additional travel costs involved in returning home are your own responsibility.
All course content is protected by copyright and is the intellectual property of Lucid Jiu Jitsu & Self-Defence. No recording or duplication of the material is permitted without express permission.
Participants acknowledge that they are attending the course and understand that they have the choice whether or not to participate in activities, role plays, exercises, drills, or any other part of the course/event.
Although Lucid Jiu Jitsu & Self-Defence takes all possible precautions to ensure the safety of all attendees, participants acknowledge that participation is completely at their own risk. Neither Lucid Jiu Jitsu & Self-Defence instructors, associates, facilitators, nor agents can be held responsible for any injury or damage to property while attending any course, workshop, training, or event. It is recommended that participants consult with a physician before participating in any form of exercise.
Results that could be achieved by attending any workshop, course, or event are provided as examples based on previous participant's achievements. Participants acknowledge their own decisions, abilities, and situations may or may not lead them to achieve the same results.
Lucid Jiu Jitsu & Self-Defence cannot be held responsible for any outcomes obtained by participants or any injury or other negative effect experienced as a result of attending an event/seminar/training now or in the future.
